What You Need For Your Complaint

Our investigations are most successful when you provide as much information as possible about the allegation and those involved. The more you tell us, the better our chance of determining whether an investigation can be pursued. Before you begin, make sure you have the following information available:

  • Name and contact information of the individual or business related to your complaint. This includes, if available, telephone numbers, e-mail addresses, etc.
  • Narrative explaining:

When did the incident occur?

Where did the incident take place?

Who was involved? (if known and applicable).

How did you come to learn about the incident in question? 

  • The name and contact information of any individual who can help corroborate your reporting information.
  • Supporting evidence in electronic format that can be uploaded with your report. This may include e-mail communications, documents, billing records, or photographs.

  • Request a call from the OIGE (Not Anonymous): If you wish to file a complaint and be contacted by a member of the OIGE, please complete the Contact Me Complaint Form
  • Submit an Anonymous Complaint: You may submit your complaint anonymously by clicking on the Remain Anonymous Complaint Form
